HOW TO READ ADULT FOSTER CARE FACILITY LISTS Comprehensive lists of licensed Adult Foster Care homes and facilities by counties are available from the Michigan Department Bureau of Health Systems (MDBHS) at the address below: Michigan Department Bureau of Health Systems Or you can call 517-373-8580 for one (1) county listing of facilities. To obtain listings for multiple counties,telephone 517-373-3598, and request these lists under the Freedom of Information. Additional information about Adult Foster Care homes is available from your local Long Term Care Ombudsman office. This list is alphabetical by the owner’s name and shows the adult foster care facility by name, address and telephone number. If a home is not on the list, it probably does not have a license. However, some homes whose license has expired or are having action taken against them by the state may not be on the list. If you think the home has a license and it is not on the list, call the above telephone number for verification. The section at the top of the page listing the Adult Foster Care homes and facilities will indicate the date the data is current as well as the date of the report and the county covered by it. The date the report is prepared by MDBHS and the county the report covers are in the upper left corner of the report. Each county has a number for state documents; the standard numerical designation of the 83 counties is the counties in alphabetical order (for example: 1 = Alcona County, 47 = Livingston County, 83 = Wexford County). The upper right corner is the page in full state report and a LAST BATCH number which is four digits. The first two are the year of the report. Across the top are the titles of the sections which are listed and described below:
If there are several homes in the same county owned by the same operator, the individual homes will be listed separately and the owner name repeated. You have to look at other counties to see if the same operator has homes in other parts of the state. Underneath the owner’s name is a line entitled: Contract Agency. If the home has a contract for an increased daily payment or special services the name of the organization may have additional information on the home.
First Column LICENSE NUMBER is a number made up of nine letters and digits. For example: SG50001100 read as SG 50 00 110 begins with two letters which refer to the size of the home. The next two digits are the county designation by number and will be the same as the county number at the top of the page. The next two digits are always "00". The last three digits are "sequencing numbers" used internally by MDBHS. Use this license number if you wish to get copies of the MDBHS licensing report or complaint investigations for a particular facility. Call MDBHS at the above number to get the reports. CAPA: the capacity, number of beds the home is licensed to operate; the home can have no more than this
number of residents
MPS ID: is the Model Payment System (Medicaid provider number) for homes which have a contract to provide personal care services. Second Column FAC TYPE: repeats the facility size listed in the License Number of the family home. (small group, large group, or congregate facility) CONS. NO: a five digit number in two groups, it refers to the licensing consultant who is responsible for licensing the facility. You can find out the name and telephone number of that consultant by calling MDBHS at the AF licensing office in Lansing. PROGRAM: always blank. The AFC licensing law allows for "programs" to serve special groups of people (elderly, mentally ill, etc.) to be certified. That part of the law has never been implemented.
Third Column The three designations indicate if the home offers a particular kind of service to the resident through a contract agency. These designations can help you figure out the home’s specialization since there is no program certification. Please note that when the contract agency is a mental health agency, the resident of the facility is protected by the recipient rights in the Mental Health Code as well as the rights guaranteed in the AFC licensing law. AIS/MR: Alternative Institutional Services for the mentally retarded. The care given is similar to nursing home care. CLF/DD: Community Living facility for the Developmentally Disabled. The home will probably have a contract with the Community Mental Board. OTHER AFE REPORTS AVAILABLE MDBHS regularly prepares several other reports on AFCs. These licenses are closed, not renewed or taken away in the previous month. These reports are available by writing to the Michigan Department Bureau of Health Systems at the address on page one.
